To GET STARTED RIGHT AWAY, please complete this CHECKLIST and simply fax it back to 904-531-3003 along with a copy of your divorce settlement agreement or divorce final judgment and the required legal language will be secured from the Plan Administrator, calculations computed and your QDRO will be completed within 72 hours and sent to your plan administrator for pre-approval.
